Any student who wants to compete in Devon Prep's athletic program is required to have an up-to-date physical.

  • Physicals must be completed after June 1, before the upcoming school year.
  • Download a PDF of the physical form. Pages 3 and 4 of the PIAA athletic form were last updated May 26, 2011, so please be sure use the 2011 version. The form can also be found at the PIAA website, www.piaa.org.
